1) The access gate to the upper area is open. The gate should only be open when in use. Also, the leg of a table is over the gate.

2) One person is in an area with no guardrail or other means of fall protection. The area should have a standard guardrail, or the employee should have another approved form of fall protection.

3) Two people are using poor ergonomic techniques. They should not be handing items to each other in this manner.

4) A cord is on the floor beneath the feet of one worker. Cords should not be run across floors, as they could be damaged or create a tripping hazard.

5) Mezzanine area. It is a safe practice to guard bare light bulbs to prevent worker contact.
