1) The order picker is too far from the shelves. The operator has to reach well beyond the running lines of the truck. This increases his chance of falling, creates a potential ergonomic hazard to the operator’s back and shoulders, and creates a danger of dropping (and possibly damaging) materials.

2) The operator has one foot on the pallet, which may cause the operator to be off-balance. Also, make certain the platform is secured to the carriage.

3) The person on the ground is between the order picker and the shelves. No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty. This is a hazard both from movement of the truck and from the potential for falling boxes.

4) The person on the ground has improper footwear. The employer shall ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects.
