1) The fall protection harness is improperly anchored to the side rail. The anchor point should be above the operator’s waist, and preferably above his head, to minimize falling distance. Also, the guard rail is unlikely to support a fall.

2) The operator is using poor lifting technique. The operator should be lifting with his legs, not his back.

3) One box is about to fall off the pallet. Only stable or safely arranged loads shall be handled.

4) A toolbox is in the aisle. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.

5) The operator is standing on the pallet, not on the operator platform. Make certain the platform is secured to the carriage.
