  1. The driver is entering the cab of a vehicle that has someone underneath it. A pre-trip inspection would prevent such a grievous error. Even if the truck is just being moved a short distance, such as across a parking area, a quick pre-trip inspection around the truck would reveal obstructions (such as tools, people, and animals). Fluid leaks or any other mechanical problems would also be caught before the problem is worsened by moving the truck even just a minimal distance. In addition, supervisors who do not normally drive need to be fully qualified to operate the commercial motor vehicle even in a parking lot open to public travel. This includes such requirements as proper licensing, training, driver qualification, hours of service, and DOT alcohol and drug testing. A trip of any kind is still seen as a trip, so the manager would need enough available hours in the 14-hour day or 60/70-hour work week to drive the truck.
  2. The maintenance worker under the vehicle is not wearing appropriate shoes. Safety shoes should be sturdy and have an impact-resistant toe. In some shoes, metal insoles protect against puncture wounds. Additional protections, such as metatarsal guards, may be found in some types of footwear.
